Appointments & Consultation
The consultation fee is $100, payable at the time of the visit (cash or check - no credit or debit cards accepted) and is applicable to the surgical fee. Scheduling may be done at the time of the consultation or at a later date per your preference. There may be a second visit required, at no additional charge, for your history and physical and the signing of the surgical consent form. Photographs will be taken at one of the visits. Your medical record is strictly private. No information will be shared or provided without your written permission.
If you are unable to keep an appointment, we ask that you kindly provide us with as much notice as possible. Surgical Facilities All procedures are performed either in a hospital operating room or at a certified surgical facility. You will find the staff at each site to be caring, cheerful and competent. We are affiliated with the following hospitals:
We will bill your insurance directly for any surgery that is covered by insurance. Please have your insurance information available for your visit. If you are a member of a managed health care plan, please be sure the appropriate referrals have been sent by your primary care provider. We cannot see managed care patients unless there is a valid referral. You are responsible for any co-payments, deductibles, or co-insurance payments. Cosmetic procedures are seldom covered by health insurance. The exact cost of any specific procedure or combination of procedures will be quoted by Dr. Gilbert at the time of your consultation. Prices are also available over the telephone. There may be additional charges for any required pre-operative testing or for any required pathological services (tissue examination). As is customary, fees for cosmetic surgery are to be paid in full in advance. We do not accept credit or debit cards. Cash or check are acceptable forms of payment. Patient financing is available through: Health Finance Options (http://www.healthfinanceoptions.com). Dr. Gilbert purchases an insurance policy for you to cover any unexpected complications of cosmetic surgery that might require additional hospital care through: CosmetAssure (http://www.cosmetassure.com). Emergencies Dr. Gilbert provides 24-hour coverage. If you have an emergency and need to be seen, please call the office at any time. |
